Health and Wellness Information:  Schedule of Preventative Health Tests by Age

Wellness Discounts: over 35,000 nationwide provides provide discounts to members of Highmark Blue Shield. This flyer gives you information on finding out if providers in your area participate.

New Hire Instructions: employee's are eligible for benefits effective 60 days from their date of hire. The corporate office must be notified within 30 days from date of hire of your intent to obtain insurance benefits. A new hire packet can be requested from your school director or the corporate office.

Open Enrollment Instructions: Employees who currently have a benefit will receive a form in November 2009 to be returned to their school directors by December 1st. Employees who wish to enroll for coverage should contact their school director to obtain an enrollment form.

For full time employees (those who work an average of more than 30 hours per week) there are Health, Dental and Vision plans available, along with Short and Long Term Disability Coverage. A Universal Life, Dependent Life and Supplemental Life insurance plans are also available. For part-time employees there is a medical-only plan available.

Employee Forms

Sage Benefit Election Form - must be completed by every employee within 30 days of their hire date.

Group Life Enrollment Form- completed by all full-time, eligible employees at the time of hire.

Medical Waiver Form- if employee elects not to enroll in medical coverage they need to complete the waiver.
